Job Summary

InfoTech Solutions is seeking a dedicated and compassionate Behavioral Health Care Manager – Remote Support to join our growing healthcare support team. This role is designed for an experienced behavioral health professional who is passionate about improving patient outcomes, supporting care coordination, and delivering high-quality remote mental health management services.

As a Behavioral Health Care Manager, you will collaborate with healthcare providers, patients, families, and interdisciplinary care teams to assess behavioral health needs, develop personalized care plans, monitor progress, and ensure access to appropriate treatment resources. Key Responsibilities

  • Conduct comprehensive behavioral health assessments through virtual platforms, telephone consultations, and electronic documentation systems.
  • Develop individualized care management plans based on patient needs, clinical history, and treatment goals.
  • Coordinate with physicians, therapists, case managers, and other healthcare professionals to ensure integrated patient care.
  • Monitor patient progress, treatment adherence, and overall behavioral health outcomes.
  • Identify patients at risk for mental health crises and coordinate appropriate interventions.
  • Provide education, emotional support, and guidance to patients and their families.
  • Maintain accurate and timely clinical documentation in compliance with healthcare regulations and organizational standards.
  • Review treatment plans regularly and recommend modifications based on patient progress.
  • Connect patients with community resources, counseling services, rehabilitation programs, and support networks.
  • Participate in team meetings, case reviews, and ongoing professional development activities.

Required Skills and Qualifications

  • Bachelor’s degree in Psychology, Social Work, Counseling, Nursing, Healthcare Administration, or a related field.
  • Master’s degree in Behavioral Health, Social Work, Counseling, or related discipline preferred.
  • Professional licensure such as LCSW, LPC, LMFT, RN, or equivalent is highly desirable.
  • Strong understanding of behavioral health conditions, treatment models, and care coordination principles.
  • Proficiency in telehealth platforms, electronic medical records, and digital communication tools.
  • Excellent interpersonal, communication, and patient engagement skills.
  • Strong problem-solving, critical thinking, and decision-making abilities.
  • Ability to work independently in a remote work environment.
  • Commitment to confidentiality, ethical standards, and patient-centered care.

Experience

  • Minimum 3–5 years of experience in behavioral health, case management, clinical care coordination, or related healthcare settings.
  • Previous experience in remote patient support, telehealth services, or managed care environments is preferred.
  • Experience working with diverse patient populations, including individuals with complex behavioral or emotional health needs.

Working Hours

  • Full-time position.
  • 40 hours per week.
  • Flexible scheduling options may be available.
  • Occasional evening or weekend availability may be required based on patient needs and operational requirements.

  • In-depth knowledge of behavioral health treatment protocols and patient care management strategies.
  • Understanding of healthcare compliance standards, privacy regulations, and documentation requirements.

  • Ability to handle sensitive situations with empathy, professionalism, and sound clinical judgment.
