Job Summary

InfoTech Solutions is seeking an experienced and compassionate Remote Nurse Case Manager specializing in behavioral health to join our growing clinical operations team. In this role, you will provide comprehensive care coordination, case management, and patient advocacy services for individuals facing mental health, substance use, and co-occurring medical conditions.

As a key member of our healthcare management team, you will collaborate with providers, patients, caregivers, and interdisciplinary teams to ensure members receive high-quality, evidence-based care while improving health outcomes and quality of life. Key Responsibilities

  • Conduct comprehensive health and psychosocial assessments for members with behavioral health conditions.
  • Develop individualized care plans based on clinical assessments, treatment goals, and patient needs.
  • Coordinate care between psychiatrists, therapists, primary care physicians, social workers, and community support services.
  • Monitor treatment progress, medication adherence, and overall health outcomes.
  • Identify barriers to care including social determinants of health, transportation, housing, or financial concerns.
  • Educate patients and families regarding treatment plans, available resources, crisis prevention, and disease management.
  • Support members during transitions of care including hospital discharge, rehabilitation, or outpatient treatment.
  • Review medical records, treatment documentation, and utilization reports to ensure compliance with healthcare standards.
  • Participate in case review meetings, interdisciplinary care conferences, and quality improvement initiatives.
  • Maintain accurate and timely documentation in electronic medical record systems.
  • Ensure compliance with state nursing regulations, HIPAA standards, and organizational policies.
  • Provide crisis intervention support and escalation when clinically necessary.

Required Skills and Qualifications

  • Active Registered Nurse (RN) license in good standing.
  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing considered with extensive experience.
  • Certification in Case Management such as CCM preferred.
  • Strong knowledge of behavioral health conditions including depression, anxiety disorders, bipolar disorder, PTSD, and substance use disorders.
  • Understanding of care coordination models, utilization management, and population health principles.
  • Excellent verbal, written, and interpersonal communication skills.
  • Ability to work independently in a fully remote environment.
  • Strong documentation, organizational, and time-management skills.
  • Proficiency with electronic health records and telehealth systems.

Experience

  • Minimum 3–5 years of nursing experience in behavioral health, psychiatric care, case management, managed care, or community health.
  • Previous experience working with high-risk populations and chronic behavioral health conditions preferred.
  • Experience in telephonic or virtual care management strongly preferred.
  • Prior utilization review or discharge planning experience is an advantage.

Working Hours

  • Full-Time Remote Position
  • 40 hours per week
  • Monday to Friday
  • Flexible scheduling based on patient and operational requirements
  • Occasional evening availability may be required for urgent patient needs

  • Deep understanding of behavioral health treatment protocols and evidence-based interventions.
  • Ability to assess patient risk and implement appropriate escalation procedures.
  • Knowledge of insurance authorization processes and healthcare reimbursement systems.
  • Strong critical thinking and clinical decision-making abilities.
  • Ability to build trust and maintain therapeutic relationships with diverse patient populations.
  • Skilled in conflict resolution, motivational interviewing, and patient advocacy.
  • Ability to manage multiple cases while maintaining clinical accuracy and empathy.
